About the ECM Program
The Enhanced Care Management (ECM) program offers a whole-person, interdisciplinary approach to care, addressing both clinical and non-clinical needs of clients with complex health and social challenges. Focus areas include:

The ECM benefit improves care coordination, integrates resources, addresses social determinants of health (SDOH), and reduces unnecessary healthcare use for high-need Medi-Cal recipients.
Job Overview
The ECM Lead Case Manager will lead community outreach, determine eligibility, and coordinate care for clients enrolled in ECM. Reporting to the ECM Program Manager, this role ensures engagement, enrollment, and consistent follow-up across clinical and supportive service programs. You will be the point of contact for client-centered care, ensuring plans align with each participant’s strengths, goals, and needs.
Key Responsibilities
· Conduct outreach and enrollment for ECM-eligible individuals.
· Complete risk assessments and develop patient-centered care plans.
· Oversee implementation and updates of care plans, reviewed quarterly at a minimum.
· Educate patients on self-management or coordinate caregiver involvement.
· Apply motivational interviewing and trauma-informed care approaches.
· Coordinate with hospitals and providers during care transitions and discharges.
· Facilitate access to services like housing, transportation, and benefits.
· Maintain at least one in-person client contact per month.
· Accompany clients to appointments, as needed.
· Document all services and encounters in the Electronic Health Record (EHR).
· Participate in interdisciplinary case conferences and internal reporting.
· Maintain flexibility to meet clients in their preferred location.
· Perform other duties as assigned.

High School Diploma required; Bachelor’s in Social Services preferred
2–3 years of experience in community health or social service settings
2–3 years of case management or care coordination experience preferred
Bilingual required: English and Spanish or Armenian
Proficient in Microsoft Office; experience with EHR systems preferred
Valid driver’s license and access to a reliable vehicle
Excellent communication, critical thinking, and organizational skills
Ability to work independently and within a multidisciplinary team
